For one low monthly fee, you can have full access to your primary care provider as often as you need. Telehealth, home visits, and office visits make appointments the most convenient option for you and your family.

Your membership is all inclusive, no deductibles necessary. Never pay a copay, ever. No ifs or buts about it.
Membership Pricing
Child 0-17: $50 (with adult member)
Adult 18-64: $135
Adult 65+: $150
Family of 4 or more: $350 (2 adults and 2+ children under the age of 21)
Employer rate per adult: $90 (minimum of 5 employees or more)
Homebound: $200 (6 home visits per year)
$75 registration fee per household
*A discount of 10% will be assessed for any membership if the entire 12 month membership is paid in advance as a lump sum.

We recommend all members have a health insurance policy or healthshare plan that will cover you for catastrophic events (aka, the 10%.)
